How to Fill In a D2 Form for HGV, C1, D1 and PCV Licences

The D2 form is the DVLA application form used to apply for a provisional entitlement for large vehicle categories, including HGV (Category C and C+E), C1 (7.5 tonne), D1 (minibus), and PCV (Category D bus and coach). If you are starting your journey towards any of these professional driving licences, completing the D2 form correctly is the first administrative step. This guide walks you through every section of the form and explains exactly what to write.

What Is the D2 Form?

The D2 is a paper application form issued by the DVLA. It is used to apply for a provisional driving licence for categories that cannot be added online, including all large vehicle categories (C, C+E, C1, D, D1, and their trailer variants). The form must be submitted by post to the DVLA in Swansea, accompanied by a completed D4 medical examination certificate.

As of 2026, there is no fully online route for adding provisional HGV, C1, D1, or PCV entitlement to an existing licence. You must use the paper D2 form. The DVLA processes most D2 applications within 1–3 weeks, though it can take longer during busy periods.

Before You Start: What You Need

Before completing the D2 form, make sure you have the following ready:

  • Your current photocard driving licence (you will need to send the photocard with your application)
  • A completed D4 medical examination certificate (completed by a registered doctor, no more than 3 months old)
  • Your National Insurance number
  • Payment of £43 (the current DVLA fee for a provisional licence application)

You can download the D2 form from the DVLA website or collect one from a Post Office that offers DVLA services.

Section-by-Section Guide to the D2 Form

Section 1: Your Details

Enter your full legal name exactly as it appears on your current driving licence. Include your date of birth, your current address, and your National Insurance number. If you have changed your name since your licence was issued, you will need to provide documentary evidence (e.g., marriage certificate or deed poll).

Section 2: Your Driving Licence Number

Enter your current UK driving licence number. This is the 16-character alphanumeric code on the front of your photocard. Double-check this carefully — an incorrect licence number is one of the most common reasons for D2 applications being returned.

Section 3: The Category You Are Applying For

Tick the box for the category or categories you want to add as provisional entitlement. The options relevant to professional drivers are:

  • C — Category C (Class 2 HGV, rigid vehicles over 3.5 tonnes)
  • C+E — Category C+E (Class 1 HGV, articulated vehicles)
  • C1 — Vehicles between 3.5 and 7.5 tonnes (ambulances, 7.5t delivery)
  • C1+E — C1 vehicles towing a trailer over 750 kg
  • D — Category D (PCV, full bus and coach licence)
  • D1 — Minibus (9–16 passenger seats)
  • D1+E — D1 minibus towing a trailer over 750 kg

You can apply for more than one category on the same D2 form, but each category requires its own D4 medical if the medical has not already been completed for that group.

Section 4: Medical Declaration

You must declare any medical conditions that may affect your fitness to drive. This section asks about conditions such as epilepsy, diabetes, heart conditions, sleep disorders, and visual impairments. Answer honestly — failing to disclose a relevant condition is a criminal offence and can invalidate your licence. The D4 medical certificate you submit alongside the D2 will confirm your fitness to drive.

Section 5: Declaration and Signature

Sign and date the form. By signing, you confirm that the information you have provided is accurate and that you understand your legal obligations as a licence holder.

What to Send with Your D2 Form

Your D2 application must be accompanied by:

  • Your original photocard driving licence (DVLA will return it with the updated entitlement)
  • Your completed D4 medical certificate (original, not a photocopy)
  • Payment of £43 (cheque payable to DVLA, or you can pay by card using the payment slip)

Send everything by first-class post to: DVLA, Swansea, SA99 1BT. Consider using Royal Mail Signed For to ensure safe delivery.

How Long Does the D2 Application Take?

The DVLA typically processes D2 applications within 1–3 weeks. During busy periods (January–March and September–October), processing can take up to 6 weeks. If you need your provisional entitlement urgently, submit your application as early as possible — ideally before you book your theory test, as you cannot sit the theory test without a valid provisional entitlement.

Common Mistakes to Avoid

The most common reasons D2 applications are rejected or delayed include: incorrect licence number, missing D4 medical certificate, D4 medical older than 3 months, missing photocard, unsigned form, and incorrect payment. Check everything twice before posting.
