D2 vs D4 Form: What Is the Difference?
If you are applying for a professional driving licence in the UK — whether HGV, C1, D1, or PCV — you will encounter two DVLA forms: the D2 and the D4. Both are required as part of the provisional licence application process, but they serve completely different purposes. What Is the D2 Form?
The D2 is the DVLA application form for adding a provisional entitlement to your driving licence for large vehicle categories. It is a form that you complete yourself and submit to the DVLA by post, along with your current photocard licence and a completed D4 medical certificate. The D2 is the administrative application — it tells the DVLA which category you want to add and confirms your personal details.
The D2 form covers the following categories: C (Class 2 HGV), C+E (Class 1 HGV), C1 (7.5 tonne), C1+E, D (PCV bus/coach), D1 (minibus), and D1+E. The current DVLA fee for a D2 application is £43.
What Is the D4 Form?
The D4 is a medical examination report form. It is not completed by you — it is completed by a registered doctor (GP or occupational health physician) after they have examined you and assessed your fitness to drive a large vehicle. The D4 records the results of your medical examination, including eyesight tests, blood pressure, heart health, and any relevant medical conditions.
The D4 medical is a legal requirement for all large vehicle categories. Without a valid D4 certificate, the DVLA will not process your D2 application. The D4 must be no more than 3 months old when you submit your application. The cost of a D4 medical is typically £50–£120, depending on the doctor or clinic.

Do I Need Both Forms?
Yes. For your initial provisional licence application, you must submit both forms together. The D2 is your application; the D4 is the medical evidence that supports it. The DVLA will not process a D2 without an accompanying D4.
When you renew your large vehicle entitlement (at age 45 and every 5 years thereafter, then annually from age 65), you will need a new D4 medical but you do not need to complete a new D2 — the renewal process uses a different form (D47).
Where to Get the Forms
The D2 form can be downloaded from the DVLA website or collected from a Post Office that offers DVLA services. The D4 form is provided by the doctor who conducts your medical examination — you do not need to source it yourself. Simply book a D4 medical with a GP or occupational health clinic and they will have the correct form.

Book your D4 medical first, as availability can vary. Once you have your D4 certificate, complete the D2 form and submit both together.
