HGV Medical Renewal Guide: When and How to Renew Your D4 Medical
Holding an HGV, C1, D1, or PCV licence in the UK is not a one-time process. Your large vehicle entitlement must be renewed at regular intervals throughout your driving career, and each renewal requires a new D4 medical examination. This guide explains exactly when you need to renew, what the medical involves, how much it costs, and what happens if you miss a renewal deadline.
When Do You Need to Renew Your HGV Medical?
The renewal schedule for HGV and large vehicle licences is set by the DVLA and is based on your age:

You should submit your renewal application up to 90 days before your current entitlement expires. The DVLA will send you a reminder letter approximately 56 days before your licence is due to expire, but it is your responsibility to ensure the renewal is completed on time.
What Does the D4 Medical Involve?
The D4 medical examination is conducted by a registered doctor (GP or occupational health physician). The examination typically takes 30–45 minutes and covers the following areas:
- Eyesight: Visual acuity (must be at least 6/7.5 in the better eye and 6/60 in the worse eye), visual field, and colour perception
- Blood pressure: Must be below 180/100 mmHg to pass
- Heart health: ECG may be required for drivers over 45
- Neurological assessment: Checking for conditions such as epilepsy, stroke history, and sleep disorders
- Diabetes assessment: Insulin-treated diabetics face additional restrictions
- General health: BMI, hearing, and any other conditions that may affect safe driving
The doctor will complete the D4 form and give you the original certificate to submit with your renewal application. Keep a copy for your records.
How Much Does the D4 Medical Cost?
The D4 medical is not available on the NHS — you must pay for it privately. Costs vary depending on the doctor or clinic:
- GP surgery: £50–£100
- Occupational health clinic: £80–£120
- Specialist HGV medical clinic: £60–£100
Some employers will pay for or contribute towards the cost of the D4 medical as part of their driver welfare policy. Check with your employer before booking.
How to Renew Your HGV Licence
The renewal process uses the D47 form (not the D2, which is for initial applications). The D47 can be downloaded from the DVLA website or collected from a Post Office. Complete the D47 form, attach your D4 medical certificate, and send both to the DVLA in Swansea. There is no fee for renewing your HGV entitlement at the age milestones — the renewal is free.
What Happens If You Miss the Renewal Deadline?
If your HGV entitlement expires, you cannot legally drive a large vehicle until it has been renewed. Driving with an expired entitlement is an offence that can result in a fine, points on your licence, and invalidation of your vehicle insurance. If your entitlement has lapsed, you will need to reapply using the D2 form (not the D47) and pay the £43 application fee.
Tips for a Smooth Renewal
